OLS-SA Intramural Basketball League

ELIGIBILITY - OLS-SA Intramurals offers age and skill-appropriate opportunities for boys and girls in grades 2^nd  through 8^th.  Members of  OLS-SA Parish or attending OLS School are eligible to participate. 

RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT FOR MINOR PARTICIPANTS  

By registering my child, I hereby give my approval for my child to participate in the OLS-SA Intramural Basketball Program.  I understand that participation in, and observation of this sport constitutes a risk to me and the child participating, and I assume this risk and release OLS-SA Intramural League, and all of its trustees, advisors, officers, sponsors, employees, volunteers, and officials from any liability therefore.  I understand that all spectators and participants must abide by the rules and regulations involving the use of the O.L.S. School and gym, which are posted in the gym.  Failure to comply with these rules could result in suspension from this basketball league. 

I know of no reason to restrict my child’s activity at OLS-SA Sports Committee events, and give my permission for participation in all activities, except as specifically noted herein.  I also give permission to the staff to treat my child for minor injuries and/or sickness.  In the event I can not be reached for an EMERGENCY, I hereby give permission to the physician selected by the League Director (or his appointed representative) to hospitalize, secure proper treatment for, and to order injection, anesthesia, or surgery for my child. 

I grant OLS-SA Intramurals and assigns, the irrevocable right and license to use video, to use photographs, to edit or crop photographs, and to use or authorize the use of such photographs or videos or any portion of them in any manner or media at any time in perpetuity, and to use My Child’s name, likeness, biographical or other information concerning him/her in connection therewith, including promotion in all media. I agree to hold OLS-SA Intramurals harmless against any liability, loss, or damage resulting from the use of My Child’s photographs, and I hereby release and discharge OLS-SA Intramurals from any and all claims in connection with such use of photographs.   

If I am unable (or fail to) provide transportation for My Child, I give permission for My Child to be driven by OLS-SA Intramurals staff and/or volunteers, including parents of participants. I assume full responsibility and I hold harmless OLS-SA Intramurals and its trustees, advisors, staff and volunteers from any and all claims related to such transportation. 

PRE-SEASON BASKETBALL CAMP FEE – Registration Fee is $15 per child for the pre-season training camp. ($10 for each additional child in the immediate family).  Make checks payable to O.L.S. Sports Committee.  The camp will run five weeks, and will be held on Saturdays starting in October.  Sessions will run for approximately an hour and a half. 

FREE OPEN PRACTICE - To assess skills, new participants should attend the open practice which will be held in October.  All volunteers, especially coaches, are asked to assist at these sessions 

STANDARD SEASON REGISTRATION FEE - Divisions are determined by the number of registrations in each age group.  Divisions are separated by gender when practical.  Games may be played on Friday evenings and/or on Saturdays.  Teams are formed towards the end of October.  Early Registration Fee (prior to 9/20/2008) is $35 per child ($30 for each additional child in the immediate family.  Regular Registration Fee (after 9/20/2008) is $45 per child, $40 for each additional child in the immediate family.  Registration includes a team tee shirt, a trophy, and team photograph.  Make checks payable to O.L.S. Sports Committee.   

REDUCED SEASON REGISTRATION FEE FOR VOLUNTEERS – OLS-SA Intramural league relies almost entirely on volunteers to make this opportunity possible for league participants.  You may opt for a reduced registration fee ($10 off Standard Season fees per child in immediate family) by posting a $20 “Volunteer Bond” as a guarantee that a parent, guardian, or approved alternate, will volunteer during their child’s games for at least eight hours (12 hours maximum per family with multiple registrations) during the season.  Volunteers will be assigned to one or more of the following duties: administrative, concessions (may include setup/breakdown), gym set-up/cleanup, Official in Charge, scorekeeping, etc…  Coaches and Assistant Coaches who attend 80% of their games automatically fulfill the bond requirement..  The check for this bond is ONLY cashed at the end of the season IF the minimum volunteer commitment has not been met.
